What we do:                        Deliver mental health services on-site at nursing homes and assisted-living communities for the underserved populations. Depending on your expertise and scope of practice, you will perform mental health evaluations, render diagnoses, develop treatment plans and coordinate patient care with the interdisciplinary team at the long-term care facility.

How we do it:                      360care contracts with individual long-term care facilities and offer you the opportunity to visit those facilities and with an established caseload.
